Bodytrack accepts a variety referral types for Exercise Physiology services, dependent on the needs and eligibility of patients.

We take a holistic approach to health and find our patients often see the best results when working with a team of health care professionals. We work collaboratively and provide regular reports and feedback to GPs and Allied Health Providers to ensure all are aware of patient treatment and progress.

Refer a Patient

Allied Health Referrals

  • We welcome referrals from all Allied Health Practitioners and are committed to communicating regularly in regards to patient treatment and progress. If you wish to discuss patient treatment in further detail before referring please don’t hesitate to contact us.

EPC - Individual Sessions

  • A Medicare rebate is available for a maximum of five services per patient each calendar year. Additional services are not possible in any circumstances.
  • Community-based patients may be eligible if they have a chronic (or terminal) medical condition and their GP has provided the following Chronic Disease Management (CDM) services:
    • A GP Management Plan (GPMP) – (item 721) and
    • Team Care Arrangements (TCAs) – (item 723).
  • Cost for Initial Assessments:
    • EPC initial assessment (60 mins) – $180 upfront ($58 rebate from Medicare)
    • EPC initial assessment (45 mins) – $122.5 upfront ($58 rebate from Medicare)
  • Cost  for Follow-up Appointments:
    • EPC  Follow-up Appointment (45mins) – $115 upfront ($58 rebate from Medicare)
    • EPC Follow-up Appointment (30mins) – $85 upfront ($58 rebate from Medicare)

Type 2 Diabetes Group

  • NB: this referral is available in addition to 5 x individual EPC sessions
  • Cost:
    • Individual initial assessment (45mins) – bulk billed
    • 8 x group classes – $25/class ($18.55 rebate from Medicare)

Work Cover

  • Bodytrack’s Accredited Exercise Physiologist’s regularly treat workers for musculoskeletal rehabilitation on a range of work related injuries.

Preferred referral process

  1. Relevant referral is written and signed by GP or CDM Nurse
  2. The patient is then informed what Exercise Physiology is and about Bodytrack. Key information to mention:
    1. Clinic location – 2/65 Sylvan Rd, Toowong
    2. Cost of sessions
  3. Fax referral and relevant medical history to Bodytrack – fax number 07 3870 4113
  4. Patient calls and books appointment or Bodytrack contacts patient to organise appointment time

When we have a referral prior to the consult we find the consult is much more valuable to the patient as we can pre-read their medical history and reason for referral and note conditions prior to appointment which allows us to spend more time prescribing exercises. We would greatly appreciate if you would please fax patient referrals to us when they are written.
