General Practitioner Chronic Condition Management Plan Exercise Physiology in Brisbane

Living with a chronic health condition takes daily effort, and you do not have to manage it on your own. If your GP has talked to you about exercise physiology, or you are looking into your options before your next appointment, you are in the right place.

Under Medicare’s General Practitioner Chronic Condition Management Plan (GPCCMP), many people with an ongoing condition can see an Accredited Exercise Physiologist with the cost partly covered. Here is how it works, and how we support you through it.

If you know it as the EPC plan, you are in the right place

You might know this as the Enhanced Primary Care (EPC) plan, or simply a care plan. From 1 July 2025, Medicare replaced the older GP Management Plan and Team Care Arrangements with a single GP Chronic Condition Management Plan. If you already hold a referral under the old arrangements, it still works. You can keep using it for allied health sessions until 30 June 2027, and the Medicare rebate is the same.

How the plan works

Your GP prepares a care plan and refers you to the allied health providers who suit your needs. A GPCCMP referral entitles you to a maximum of 5 Medicare-subsidised allied health sessions in total each calendar year, shared across all the allied health providers on your care plan. Depending on your condition, you can be referred to more than one type of allied health provider.

Your GP decides whether the plan is right for you. It is designed for people living with a chronic medical condition, which Medicare describes as one that has been present, or is likely to be present, for six months or longer. That covers a wide range of conditions, for example asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke.

How we help

Our Accredited Exercise Physiologists (AEPs) are university-qualified and ESSAM accredited. We use individually prescribed exercise to help you manage your condition and feel better in how you move and function day to day. Exercise is medicine, and for many chronic conditions it is one of the most evidence-based ways to feel stronger and stay well.

If you are living with Type 2 diabetes, there is a little more available to you. You may be able to access 8 group classes in addition to an individual GPCCMP referral, subsidised by Medicare. These are separate from the 5 individual sessions and have their own referral requirements, so it is worth asking your GP whether this applies to you.

How many exercise physiology sessions do I get under the GPCCMP plan?

Do I need a GP referral to use the GPCCMP plan for exercise physiology?

Yes. Your GP prepares a Chronic Disease Management care plan and refers you to the allied health providers you need. You bring that referral to your first appointment with us. Without a valid referral, Medicare does not subsidise the session.

Will I have anything to pay out of pocket?

Medicare pays a rebate toward each session, and you pay the gap between our fee and the rebate. With an Accredited Exercise Physiologist the out of pocket cost is $135 for your initial 45 minute assessment, $130 for a 45 minute follow up, and $95 for a 30 minute follow up. Sessions with a Senior or Principal Exercise Physiologist are a little higher, and our full fees are on our pricing page. Can I use my GPCCMP plan for a telehealth appointment?

Yes. Our GPCCMP exercise physiology sessions are available in clinic at Toowong and Sherwood, and via telehealth. Telehealth is delivered through Cliniko, so you can see us from home.

I have Type 2 diabetes. Are there extra Medicare services for me?

Yes. If you are living with Type 2 diabetes, you may be able to access 8 group classes in addition to an individual GPCCMP referral. These are separate from your 5 individual sessions and have their own referral requirements. Ask your GP whether this applies to you.

To refer a patient to us for exercise physiology, please include the care plan and referral details and send them to our Toowong or Sherwood clinic.
